Information on fees:

PPO insurances: Our office will bill your insurance as a non-contracted PPO provider. We will collect 30% of the total amount of the visit or the full amount of the visit (if the deductible has not been met) after services are rendered on date of service. We will also submit the bill to the insurance for the full amount and any unpaid balance will be billed to the patient.

Underprivileged Children’s Clinic and Straight Medi-cal: This clinic is for Children with financial hardship, without insurance, or patients with straight Medi-Cal. If you have Medi-Cal HMO (with a plan) please contact your Medi-Cal HMO Provider or Pediatrician for proper care. Please call our office to schedule an appointment, do not schedule online.

Medicare: Our office is contracted.

Cash Price: New patients office visit will be $275.00.

Refraction: For all insurances (PPO, Medicare), we will be collecting a Refraction fee of $45 if the doctor deems it necessary after services are rendered on date of service. Additionally, the patient will be responsible for any non-covered services as deemed by the insurance.

Cancellation: If you are unable to make it to your scheduled appointment time, please notify us at least 24 hours in advance to cancel or reschedule. Otherwise, there will be a $50 charge billed to your account for no shows.
