Directions for booking an appointment:
1. Choose appointment type & location (home or office). Prices for consults are listed when choosing appointment type. Home consults may incur a travel fee and/or convenience fee.
2. Answer all the questions when booking your appointment (Cancellation, Terms & Conditions, Travel Policy, etc.) and then there will be a link to the Mobile Lactation Consult app which will ask for you for more detailed medical information about you and your baby/babies. Please do that immediately. If you have any tech problems, please let me know.
3. Cancellation: any payment made will be refunded as long as a request is made with more than a 24-hour notice.
4. Please read the email confirmation carefully because there are important directions included and always check your email as that is how I communicate.
Home Consult Note: The appointment start time you choose may need to be adjusted to account for travel time. We will contact you if that is the case. If I am running more than 15 mins behind due to travel conditions, I will call.
Insurance Coverage: I am in-network with Aetna only, and ConnectiCare typically covers one home visit within the first 3 months after delivery. You are responsible for checking your eligibility ahead of time for specifics. If you get a pre-authorization for my services, you are still responsible to pay at the time of service. Aetna Clients: Please choose appointments only from that list.
Payment: Payment for your consult is due in full either when booking the appointment or at the time of the appointment. The only exceptions are Aetna clients. You may use a cash, check, credit card or HSA card.
If you are not able to find a suitable time, please email or call us at 860-255-8583 so that we can see if there is an alternate available time slot not visible on your end. We will try our best to fit you in as soon as possible.
