Online booking is for HGV/PSV and Taxi/Private Hire/PCO/TFL medicals only at a cost of £55. For all other medical services please contact us via phone or email.
Please read the following information before booking
We are based in Hall Road Clinic, Aveley, RM15 4HD
You will need to bring with you to your appointment:
1. Photographic ID - preferably Driving License or Passport
2. The Name and Address of your GP surgery
3. Your Medical Form for completion e.g. D4, Taxi form
4. Glasses if worn (Reading and Driving glasses Inc.). If you wear contact lenses please bring a spare set, or your glasses, as you will be tested with and without your lenses.
5. A list of Any Medication that you take regularly and details of active or recent hospital appointments and tests.
Transport for London Medicals (TFL) ONLY - It is a requirement to produce a printed copy of your full medical records/emailed copy or applied to your NHS App are no longer acceptable - our doctors will see these at the time of your examination. These can be obtained from your GP. This applies to TFL medical licensing only. If these are not available at your examination, your appointment will be cancelled and you will need to return with them on another date. This will result in a delay or your medical form being completed.
ENG1 Medicals (Seafarers) only - You must have the NHS app downloaded to your phone and this is to be accessible to the appointed doctor on the day with a view for your full medical medical records. Any previous certificates held for past ENG1's must be surrendered at your appts.
Cancellation Policy: If you need to cancel or reschedule your appointment you will need to give our office 48hrs notice prior to your appointment by calling 01708 865128 during our office hours. If you cancel or reschedule into the 48hrs prior to your appointment you will be charged 1/2 of the fee. If you cancel or reschedule on the day of your appointment then you will charged your full fee and no refund will be given. By continuing with this booking you are agreeing to the terms of this policy.
A confirmation email with further information will be sent to you after booking. Please read this email carefully prior to attending your appointment.
