Mini Nutrition Assessment
A short visit to meet your dietitian privately, or with family present. We'll screen for any areas of concern, and provide a brief report with nutrition & hydration recommendations afterwards.
Full Nutrition Assessment & Care Plan
Recommended whenever nutrition-related issues start or change. Together with your Resident Care Manager and family (if desired), your Dietitian gets a fuller understanding of your nutrition status and needs. We discuss any concerns, and create a nutrition and hydration plan together.
Resident's Followup Appointment
This may be a one-time thing, as we continue the conversation from the Full Assessment, or an ongoing relationship where your Dietitian checks in on you on a regular basis. Either way, we continue to educate and support you and your team (including family if desired) in followup care. We'll adjust your nutrition and hydration plan to better match your current needs, and make referrals if needed.
Brightwater Staff's First Personal Nutrition Consult
An hour to ask your burning questions regarding your own health and lifestyle, or to get a full assessment and advice on what you can tweak to better reach your health goals. Besides recently specializing in older adult nutrition, Nicole has years of experience supporting adults with all the most common reasons to see a dietitian: chronic disease prevention & management, digestive concerns & food restrictions, disordered eating, and meal planning for singles, couples & families.
Brief Staff Consult or Followup
Use this option to book dedicated time to discuss and get advice on resident's nutrition and hydration (individually or as a group), or to get further advice and support on your own nutrition, after an initial private consult.
