When you book your appointment please note if you are doing an individual or couple appointment, and note the session time length. You must also select your timezone before booking an appointment.

When you receive your confirmation email for this appointment, you will receive another encrypted email with forms I will need you to fill out and provide either before or during our first appointment. This is only required if this is your first appointment. This is not required for consults.
The forms you will be asked to complete are fillable PDF files which must be downloaded to your computer in order for you to fill them out. Please save them with your name in the file name.

Please download forms to your computer. Open them with adobe acrobat, not in a web browser, then fill them out, and save them with your first name in the file name on your computer.
I will have you submit them directly to me through encrypted email before we meet for our first appointment.
Do not fill in your credit card number, expiration date or security code. I will ask for those when we meet.
