HOW TO SELECT A VISIT TYPE
-In Person Visit: Select this option if you see our providers in Bend.
-Remote Visit: Select this category if you prefer to see our providers via video or phone visit.
-VIDEO visits: Select this option for Telemedicine (can be billed to insurance for local patients)
-PHONE visits: This is a cash-pay option for those who live out of state, where our scope of practice is more limited (Paid at time of booking, cannot be billed to insurance)
-NEW Patient: Select this if you have never been to Wildflower Natural Medicine, or it has been over 3 years.
-RETURN Patient: Select this if you have seen any of our providers in the past 3 years

Insurance: Insurance can be billed as a courtesy for local patients, but it is your responsibility to know your insurance coverage. You are responsible for any out of pocket costs such as copays and deductibles. Please use our Insurance Verification Form to learn the details of your plan. *Note: We are taking limited medicaid patients per month. You may be contacted to reschedule.
