DO NOT schedule an appointment if within the past 14 days:
- you or a family member has traveled outside of Canada; or
- have had close contact with a person who was suspected or confirmed to have COVID-19.
DO NOT schedule an appointment if you have flu-like symptoms such as:
- fever;
- cough;
- difficulty breathing;
- chest pain or pressure;
- fatigue; or
- loss of taste.
DO NOT BRING CHILDREN OR FRIENDS/FAMILY WITH YOU TO YOUR APPOINTMENT.
- you MUST leave your CHILDREN at home;
- do not bring FAMILY/FRIENDS with you to the appointment unless you need a TRANSLATOR or support if you have PHYSICAL LIMITATIONS;
- if several family members need to fingerprinted, please only have one person come up to the office at a time; and
- come to your appointment at your scheduled time (please do NOT come early or late).
PAYMENT METHODS: please pay by debit or credit card (we prefer
NOT to be paid by cash).
BY APPOINTMENTS ONLY: our office is operating by appointment only at this time (no walk-ins).
