WE STRONGLY SUGGEST YOU WATCH THE VIDEOS AND FAMILIARIZE YOURSELF WITH THE FAQ's AND DETAILS REGARDING EACH SERVICE. IMPORTANT INFO IS INCLUDED IN ALL, INCLUDING BUT NOT LIMITED TO HAIR DYE AND BATHING SUITS IN THE FLOAT SUITE, LIGHT SENSITIVITY IN THE LED, BEING ON TIME FOR APPOINTMENTS, ETC.
Gift Certificates. You will have the option of printing your own gift certificate from home or email it to the lucky recipient. And of course you can always stop by the Centre and pick one up in person.
All Gift Certificates expire 5 years from the date of purchase. After 12 months from issue date, Revitalize reserves the right to assess an annual fee of 10% (inactivity fee) for any part of the Certificate not used. Notice of fee due will be sent to the email associated with the Certificate. If this fee is not paid within 5 business days of email the Certificate is categorized as “unclaimed” and rendered null and void. There is no reimbursement due from seller to Certificate holder if the Certificate becomes unclaimed. Please understand your rights and contact the PA Office of Attorney General’s Bureau of Consumer Protection at 1-800-441-2555 or online at www.attorneygeneral.gov if you have any questions or concerns.
All multi-session packages expire 1 year from date of purchase.
