TO PROVIDE OUR CLIENTS WITH THE PERFECT EXPERIENCE WE MUST STAY ON SCHEDULE. YOUR APPOINTMENT TIME, SAY 2:00, MEANS YOU START YOUR SESSION AT 2:00. WITH THIS IN MIND IT IS NECESSARY FOR YOU TO BE IN THE LOBBY READY TO GO WHEN YOUR APPOINTMENT TIME ARRIVES. WE ASK THAT YOU BE HERE 10 MINUTES EARLY TO FILL OUT PAPERWORK, PAY BALANCES, USE THE RESTROOM OR BETTER YET, RELAX. PLEASE UNDERSTAND THAT COMING IN THE DOOR AT 1:57 FOR A 2:00 APPOINTMENT WILL NOT WORK. FAILURE TO ADHERE TO THIS REQUEST WILL RESULT IN A CANCELLATION.
WE STRONGLY SUGGEST YOU WATCH THE VIDEOS AND FAMILIARIZE YOURSELF WITH THE FAQ's AND DETAILS REGARDING EACH SERVICE. IMPORTANT INFO IS INCLUDED IN ALL, INCLUDING BUT NOT LIMITED TO HAIR DYE AND BATHING SUITS IN THE FLOAT SUITE, LIGHT SENSITIVITY IN THE INFRARED BED, BEING ON TIME FOR APPOINTMENTS, ETC.
All Gift Certificates expire 5 years from the date of purchase. After 12 months from issue date (and each annual issue date following), Revitalize reserves the right to assess an annual fee of 10% (inactivity fee) for any part of the Certificate not used. This fee will be applied at the time you use the Certificate. If this fee is not paid the Certificate is categorized as “unclaimed” and rendered null and void. There is no reimbursement due from seller to Certificate holder if the Certificate becomes unclaimed. Please understand your rights and contact the PA Office of Attorney General’s Bureau of Consumer Protection at 1-800-441-2555 or online at www.attorneygeneral.gov if you have any questions or concerns.
