Some insurances (especially Community Health Group) and all Employee Assistance Programs (EAP) require pre-authorization. Please check with them to obtain an authorization if needed. If you need a translator, please call your insurance plan and a translator can be arranged. Click on the staff to see their schedule, then select the appointment time. Click Pay Later if you are using insurance or EAP.
