REQUIRED FORMS MUST BE FILLED OUT FOR ALL CURRENT AND NEW CLIENTS. THEY ARE AS FOLLOWS:

*COVID-19 Consent Form should be filled out before every appt
*Wax Consent Form (if you are booking for any wax service)
I want to make sure that the area is safe and cleaned properly and sufficient. Certain precautions will be taken. I will be providing your mask that is required for you to wear during any service. I ask that you do wash your hands before your appointment begins.
LASH COVERAGE:
If you are missing more than 40%-50% of your lashes on both eyes, you will be charged for a FULL SET!
REMAINING BALANCE:
Remaining balance will be due the date of your visit. You also, have an option to pay for the service in full if you’d like. You are required to save your card on file. I ONLY ACCEPT SQUARE, and OR ZELLE . If paying remaining balance with CASH please be sure to get cash prior to your appointment, with the exact amount.
LATE FEE POLICY:
In the event that you are 10 minutes late to your appointment your deposit will be used as a late fee & FULL payment will be due once you arrive to your scheduled appointment. PLEASE BE ON TIME.
If You are 15 minutes late to your appointment you will be required to reschedule, and will have to REPAY the NON-REFUNDABLE DEPOSIT!
NO CALL NO SHOW:
If you have 2 “no call no show” your information will be blocked from booking any future appointments.
CANCELLATION POLICY:
A 24 hour notice is required in the event of any cancellation. Cancellation fee $75 will be charged if you do not cancel your appointment before the 24 hour grace period. ***Uncontrollable circumstances will be considered**
NO EXTRA PEOPLE OR CHILDREN:
No children / extra people allowed my goal is to provide the most professional atmosphere possible. Also, to keep you all safe!
EARLY & AFTER HOURS APPOINTMENTS:
After hour appointments are available upon request only (Mon & Wed) if needing to book appointments outside of scheduling hours there will be an additional $50 fee. House Calls are subject to artist if available please note an additional $75 fee. You will have to text my business line to book these appointments 214-755-0833
