1. Please choose the category of appointment you are making from the list below. A box will pop up for you to "ADD TO YOUR APPOINTMENT"

2. Please check all of the boxes for any additional fees for Travel, After Hours, Holidays or Emergencies, Large Animal Fees or Keep Sake Remembrance Items. If Add on fees are not selected and/or if additional services are provided we will invoice you after your appointment via email.

3. There are no Refunds for Cancellations with less than Four (4) hours notice.

Occasionally a prior appointment can take longer than we anticipate, please allow a grace period for arrivals.

For heavier pets we may ask for your help with our stretcher into the back of our vehicles.
