3. In-Person Physical Exams

1. In-Person CDL/DOT Physical

Head to toe physical performed by a Registered Medical Examiner/Nurse Practitioner. This exam qualifies you for a CDL.

Medical exam as part of the qualifications to drive for Lyft/Uber

4. In-Person Employment/Sport Physical Exam

Medical exam for employment or sports requirements. Please bring a blank form from your organization to the exam.

New patient appointment for hormone assessment and treatment options. Appointment will be done via tele-health and patient will go to lab of their choice to have blood drawn. Up to one hour in length.

Tele-Health Established Patient - Hormone Followup and Treatment

30 minutes @ $100.00

Tele-health hormone followup and treatment. In order to properly address hormone issues, new labs need to be drawn every 3-6 months. This appointment allows us to follow your hormones and keep you feeling your best. Up to 30 minutes in length.

Tele-Health New patient appointment with mental health concerns such as anxiety and depression. We will discuss all symptoms and health issues and devise a plan of care including micronutrient supplementation. Up to one hour in length.
