Foundational Care Plans - Terms and Conditions

By signing up for a foundational care plan, you agree to the following terms and conditions:

Individual Use Only: The selected care plan is valid only for the individual who signs up and cannot be transferred to another person.
Session Usage:
- Foundational Care Plan #1: Fifteen sessions must be used within 125 days from the date of purchase.
- Foundational Care Plan #2: Ten sessions must be used within 85 days from the date of purchase.
- Foundational Care Plan #3: 5 sessions must be used within 55 days from the date of purchase.
- Unused sessions will expire after the specified time frame.
Cancellation and Rescheduling: If you need to cancel or reschedule a session, please provide at least 24 hours' notice. Sessions can be rescheduled until 12 hours before the start time of the appointment. Failure to do so may result in a forfeiture of that session.
No-Show Policy: If you do not show up for a scheduled session without prior notice, that session will be considered used.
Non-Refundable: All payments for foundational care plans are non-refundable. Once a plan is purchased, the payment cannot be refunded under any circumstances, unless a refund is requested within 24 hours of Purchase.
Modification of Terms: These terms and conditions may be subject to change. You will be notified of any updates.
Governing Law: This agreement shall be governed by the laws of the State of Pennsylvania.

Monthly Care Plans - Terms and Conditions

By signing up for a monthly care plan, you agree to the following terms and conditions:

Individual Use Only: The selected care plan is valid only for the individual who signs up and cannot be transferred to another person.
Session Usage:
- A minimum commitment of 3 months is required for all monthly care plans, unless you enrolled in a community plan directly following the completions of a Foundational Care Plan.
- You have the option to switch between Monthly Plan #1 and Monthly Plan #2 during this period, with prior notification.
Cancellation and Rescheduling: If you need to cancel or reschedule a session, please provide at least 24 hours' notice. Sessions can be rescheduled until 12 hours before the start time of the appointment. Failure to do so may result in a forfeiture of that session.
No-Show Policy: If you do not show up for a scheduled session without prior notice, that session will be considered used.
Non-Refundable: All payments for monthly care plans are non-refundable. Once a plan is purchased, the payment cannot be refunded under any circumstances, unless a request is made within 10 days of the first payment made following the completion of a Foundational Care Plan.
Modification of Terms: These terms and conditions may be subject to change. You will be notified of any updates.
Governing Law: This agreement shall be governed by the laws of the State of Pennsylvania.
