MPF PASSPORT APPLICATION QUICK INSTRUCTIONS
Step 1. Electronic Foreign Clearance Guide (FCG), https://www.fcg.pentagon.mil/fcg.cfm The military member deploying or PCSing could check on some of the requirements for the country they are travelling to prior to submitting the application.
STEP 2 - Log on to the State Department website: www.travel.state.gov
The online application is mostly self-explanatory. When you navigate through the Website, you will come across “Passport Products and Fees”, Select the “Passport Book Fee” box then click “Next”. You will then scroll down to the bottom of the page and see an acknowledgement statement which reads: I have read and acknowledge the steps and information contained above. Click the square box and go to the “CREATE FORM” box. Within the CREATE FORM box click on the CREATE FORM link then open and print application on separate sheets (NO FRONT TO BACK/ 2 SIDED PRINTOUTS ALLOWED). A barcode will be generated in the upper left corner of the application when completed correctly.

-Original marriage license, for spouse request, No-fee passport. (if name has changed due to marriage, divorce, etc.)
-Proof of Citizenship: Original birth certificate or current/expired US passport (issue date within 15 years), form FS-240 (Report of Birth Abroad), Form DS-1350 or FS-545 (Certificate of Birth issued by the Department of State), or Certificate of Naturalization.

SPECIAL REQUIREMENTS FOR CHILDREN
“AS DIRECTED BY PUBLIC LAW 106-119 AND 22 CFR 51.27”: To submit an application for a child under age 16 both parents or the child’s legal guardian(s) must be present at the passport appointment and present the following:
-Evidence of the child’s U.S. citizenship.
-Evidence of the child’s relationship to the parents/guardians, AND
IF ONLY ONE PARENT APPEARS YOU MUST ALSO SUBMIT ONE OF THE FOLLOWING:
-Second parent’s notarized written statement consenting to passport issuance for the child (DS-3053) *Written consent from a non-applying parent for a minor under 16 must be less than 3 months old and accompanied by a photocopy of the non-applying parent’s identification OR
-Primary evidence of sole authority to apply
SPECIAL REQUIREMENTS FOR CHILD SUPPORT
Section 51.70(a)(8) of Title 22 of the Code of Federal Regulations states, in part: If an applicant is identified to Passport Services by the U. S. Department of Health and Human Services (HHS) to be in arrears of child support payments in excess of $2,500, they are ineligible to receive a U.S. passport. If this applies to you, Passport Services strongly recommends that you contact the appropriate State child support enforcement agency to make payment arrangements before applying for a passport. Please direct any questions to the appropriate State child support enforcement agency.
For Members Going TDY
Please make sure you have TDY orders with you when you are coming to process passports. If you are an active duty military member, please make sure you check the Foreign Clearance Guide (https://www.fcg.pentagon.mil/fcg.cfm) to make sure that the country that you are going to requires a passport. Not all countries require passports for active duty military personnel.
For Members PCSing
We CANNOT process passports until your dependents are medically cleared and your have published orders. If you are active duty Air Force PCSing, Outbound Assignments will sign off on your “Passports” item on your vOP Checklist.
Members that are PCSing to a location that require diplomatic passports have to have authenticated orders before they can apply for the passport. Please make sure you have orders in hand and other required documents at the time of your passport appointment.
If you are going to a country that requires a VISA, you must apply for an official passport first. You cannot apply for a VISA until the official passport comes back. Once the official passport comes in, you will be able to pick it up and start the VISA application process. Please make sure you have enough time for both the passport and VISA to be processed.
