HMO PLANS REQUIRE A REFERRAL FROM YOUR PRIMARE CARE PHYSICIAN
On your first visit please arrive 15 minutes prior to your scheduled time to complete your paperwork.
Bring your insurance card and a picture ID. Any previous imaging (X- Rays, MRI's, CT scans, etc) may be helpful.
