The consultation includes an assessment of your pre-existing condition, the risks and benefits of medical marijuana aligned to your symptom and an overview of the Ohio medical marijuana program.

The $199 all-inclusive fee covers the following:

* Patient Evaluation
* Medical Marijuana Recommendation,
* All requisite follow-ups throughout the year
* Placement into the Patient Registry.

*FOR EXISTING MEDICAL MARIJUANA CARD HOLDERS IN NEED OF RENEWING THEIR EXISTING RECOMMENDATION*

- If you're not already an OGT patient, please send copies of your medical records from your original consultation.

- All existing OGT patients medical records will already be on file, no additional documentation is required.

During the consultation, the CTR Physicians will review your approved symptom(s) along with your cannabis treatment to ensure it is still a viable option.
