Please be prepared with and any medical records and all necessary documents (a valid state drivers License or State ID with CURRENT address). Without the necessary documents, we will not be able to proceed with your consultation. If further documentation is required, records can be requested by the Physician. You may FAX records to: 412-275-5798 Digital records can be uploaded on your medical intake form.
*Also, please note the cost of the appointment is listed on your appointment type, and for Teleconsults, a credit card is required for the doctor fee. A credit or debit card is also required to pay the state for your card online. Teleconsults require preparation prior to the appointment. A staff member will call you 1-2 days PRIOR for 1) Records, 2) Registering or two-step online renewal on DOH portal, 3) payment, and 4) medical intake history.
