COVID/Mask Policy
If you are seeing Brooke in office, masks are encouraged to be worn as this is a medical facility where sick patients are being cared for. For the health and safety of Brooke so that she can continue seeing other families, if you are feeling sick or unwell, please reschedule or wear a mask at the time of your visit.
Office Locations:
Brooke has 2 office locations and is available at those offices on different days. Please be sure you are choosing the right location for where you would like to schedule your visit!
-Sunshine Pediatrics location at 210 Lookout Place Maitland, FL 32751
-Heart 2 Heart Birth Center location 1110 Lexington Green Lane Sanford, FL 32771. If you are wanting to schedule at the birth center, please reach out to Brooke directly by email or call (DO NOT TEXT!) for an appointment. There may be more availability for this location that does not show on the availability calendar!
***All visits are subject to confirmation. In the event that your visit time needs to be rescheduled, you will contacted as soon as possible by phone so that a new time can be scheduled!***
Driving Fees for Home Visits:
Due to the rising cost of gas, a driving fee will now be added to any home visit that is more than 25miles from Brooke’s Maitland Office Location. The address is 210 Lookout Place Maitland, FL 32751. The fee will range from $10-$30 depending on your location. You will be notified once Brooke confirms your appointment. *Please note….Brooke’s main office is in Maitland. Any home visit that is not within 25miles of the Maitland office WILL have a driving fee.*
Cancellation Policy:
Please call with 24 hours notice if you need to cancel or reschedule your visit. You will be charged a $50 fee if your visit is cancelled with less than 24 hours notice. If you fail to show for your visit with no notice, you will be charged for your visit in full.
Late Policy:
If you arrive more than 15min late for your office appointment, your appointment will be rescheduled. Please contact Brooke as soon as possible if you think you will be late for your visit so your options can be discussed.
