By booking this appointment, you agree to the following terms and conditions:

• I understand that if I am feeling ill, showing symptoms of COVID-19, have/recently had COVID-19, and/or have been exposed to COVID-19 within the past 14 days, then I must cancel/reschedule my appointment for 14 days out.
• I understand that none of the treatments offered shall be performed if I am menstruating or pregnant at the time of my scheduled appointment. And if I become pregnant or begin menstruating then I shall seize services and inform Southern Belle Body Contouring, LLC.
• For Vaginal "Peach Steams", I understand that if I steam with an IUD that there is a small chance that the IUD could dislodge and no longer be an effective form of birth control. It is also advised to wait to steam for 3 days AFTER vaginal waxing/shaving.
• For Body Contouring, I UNDERSTAND FINAL RESULTS TAKE 72 HOURS and multiple sessions may be needed for maximum results! I also understand that during those 72 hours after each session, I must follow the after-care instructions (if receiving Cavi Lipo) for optimal results:
  • Do NOT drink alcohol 72 hours after service
  • Do drink plenty of water
  • Exercise
  • Eat clean

• I understand that if I do not follow instructions specifically, then Southern Belle Body Contouring, LLC is not responsible for the results.

• A $25.00 non-refundable deposit is required at the time of booking for all services unless otherwise noted. Deposit does not apply to consultations.
• Peach Steams require full payment at the time of booking. It's a Peach Party requires a deposit amount of $50.00 at the time of booking. .
• Client will be considered "no-show" if more than 15 minutes late and will have to book a new appointment or may be charged a $25.00 late fee.
• Payments received will be forfeited for no-shows and/or appointments not cancelled at least 2 hours prior to appointment start time.

• I understand that this is a cosmetic treatment and that no medical claims are expressed or implied.
• I give permission for photographs to be used by Southern Belle Body Contouring, LLC and staff for monitoring my treatment progress and promotional purposes.
• I give consent to receive treatments from Southern Belle Body Contouring, LLC and hereby release Southern Belle Body Contouring, LLC and its employees against any and all liability associated with treatment(s).
