If this is your first appointment, please be sure to arrive at least 15 minutes early to complete your Client Health History Assessment. To save time, download at home and bring the completed forms with you to your appointment.
Please be on time for your appointment. If you are late, we may not be able to accommodate your full session.
We require 24 hours notice for cancellations. Failure to cancel more than 24 hours in advance, or if client does not show for an appointment, client may be required to pay for their appointment in advance. If a client fails to show and or cancels with less than 24hrs notice a second time, the client’s credit card will automatically be billed for the full amount of session scheduled.
