The health history form is designed for adults - so please only fill out what seems applicable and skip the rest. We will have plenty of time to talk and get to all of the pertinent details.

Each Lumination is a group session with Laura Washington ND that you join either by phone or an online link through Zoom. Luminations shed light on you, your health, and your process of living. They will calm, steady, and fortify your nervous system. They support you in finding inner clarity and and living more fully and joyfully in the current moment. After signing up you will receive information on joining live. On completion of the live event a recording will be sent.
