Welcome to the City of Houston, Municipal Courts Department, US Passport Acceptance Facility Scheduler. We can only accept the DS-11 passport application. We cannot accept the DS-82 renewal application, it must be completed and mailed by the applicant to the address on the form. This can be found on the US Department of State website at www.travel.state.gov.
To get started, choose Acceptance location from below to view available times and schedule your appointment. A separate appointment is needed for each passport applicant, including minors. After your appointment you will receive a follow-up email that will contain a link to an optional customer experience survey; data collected will only be used to improve services offered by the Houston Municipal Courts Department.
Please read below for important information needed to successfully complete your passport appointment.
MUST HAVES for all applicants:
1. Correctly Completed typed or handwritten DS-11 Application for a US Passport with no mistakes or cross-outs - Download application online at travel.state.gov (Use only black ink)
- Answer Questions #1-21 (read carefully)
- Do not sign application until requested to do so by a passport acceptance agent. If corrections and/or additions are needed a new DS-11 will have to be completed on site.
- Parental information should match the applicant’s birth certificate. Include the City and State (or Country if applicable) of parent’s birth.
- FOR MINOR APPLICANTS on address line 2 include: "IN CARE OF" [insert a guardian’s first and last name]. Do not write the word “Parent”. Parental requirements for minor applicants can be found here: Children Under 16

2. Proof of U.S. Citizenship
- Bring original (plus one photocopy) of the following:
- Previously issued undamaged U.S. Passport (may be expired)
- U.S. Birth Certificate or certified copy issued by the city, county, or state
- Certificate of Naturalization
- Consular Report of Birth Abroad or Certification of Birth
- Certificate of Citizenship
- For minors, the original birth certificate must be presented every time an application is submitted.
- Minors with a previous passport must submit the original or a copy of their birth certificate along with the expired or current valid passport.
Note: Original proof of citizenship must be submitted with your application, however, the original document will be returned to you by mail in a separate envelope. - Link: Eligible Citizenship Documents
3. Proof of Identity - Bring original (plus one photocopy front and back) of the following:
- Valid Driver’s License
- Valid State Identification Card
- Naturalization Certificate (picture must be a true current likeness of citizen)
- Prior Undamaged U.S. Passport (may be expired; picture must be a true current likeness of citizen)
- Government Employee ID
- Official Military ID Card
- For a minor under 16 years of age, both parents must be present? If not, please complete the DS3053 for parental consent for the non-appearing parent. This form has to be completed correctly and notarized.
- Out of State applicants must present an additional secondary ID (i.e. Work/School ID, SSI Card)

4. Photo - Passport Photo (Photo services are not provided at this facility)

5. Payments Two Payments Are Needed Payment 1 of 2: Passport Application Fee (Exact Amounts: No Overpayments or Underpayments.) - IMPORTANT: Passport application fees can only be paid by personal check, cashier’s check, or money order
- Personal checks must have pre-printed name and address without corrections. Temporary/starter checks are not accepted.
- Separate payment for each applicant
- Made payable to U.S. Department of State (cannot be abbreviated)
- Cannot be made in cash, or credit/debit cards
- Non-refundable
And Payment 2 of 2: Acceptance Fee - $35 per applicant
- Paid separately to the Acceptance location
- Not included in the Passport Application Fee
- Acceptance fee can be paid in cash, check, credit/debit card, money order, or cashier’s check.

Appointments are 30 minutes in duration and are available Monday through Friday, between the hours of 1:00 PM to 10:00 PM. Please arrive at least 10 minutes before your scheduled appointment. Monitor your email for appointment confirmation and important appointment updates. Text notification option: A reminder text will be sent to the cellphone number provided within 24 hours of your scheduled appointment. Please note your carriers' standard text messaging rates apply. |
