1. Your coach agrees to treat all information shared during sessions as private and confidential. However, your coach is not bound by any professional confidentiality agreements, and may from time-to-time, anonymously share information from sessions with trainers or other coaches for feedback.

2. The Coaching process is a powerful process that will be a combination of empathetic counseling and your own willingness to grow. As with anything, the more you give to this process, the greater your benefits.

3. You will have 60-minute sessions via telephone scheduled in advance to occur weekly. You will also receive unlimited e-mail/text coaching accessibility.

4. The best results are incurred when you attend weekly sessions with your coach, do homework as assigned, and take advantage of the unlimited e-mail/text coaching benefit.

5. At the scheduled time, you will call your coach at 414-345-0791 unless requested to do otherwise.

6. In the event that you must cancel your scheduled coaching session, a 24-hour notice is appreciated. If you miss your scheduled session and have failed to give notice, you will be charged in-full for the session.

7. The fee schedule for this coaching relationship is $100 per session and we have an hour session each week and unlimited text and e-mails in between. You can pay by PayPal using the Acuity Scheduling website. I charge by the month, so the total for each month is $400, and session packages (4 sessions total) will be required to be used with 45 days of purchase.

I understand that Coaching is not advice-giving, psychotherapy, or counseling. I agree to seek these or other professional services, if needed.
I understand that I am fully responsible for the decisions and actions I take in regards to my life and affairs.
I agree to be mindful of my own well-being during the course of this coaching process.
I shall in no way hold the coach liable or responsible for any actions I take during or after this coaching relationship.
I understand that all coaching materials and documents shared with me as a part of this process are proprietary information from Erika Reith and Erika Reith Coaching, and may not be used in any way outside of the context of this coaching relationship.
I understand that the coach makes no guarantees or warranties, expressed or implied, about any results to be achieved.
I understand that I may at any time disengage from the coaching relationship and that my coach may do so, as well. Upon receipt of a written statement saying you wish to quit your coaching work, we will terminate the coaching relationship immediately. By doing so, I understand that I will forfeit any remaining balance without a refund for services either rendered or unused within my package.
By agreeing below, I acknowledge that I have had the opportunity to review this agreement, that I understand all aspects within it, and I agree to abide by it.
