For original permit applications please have the applicable documentation for the type of permit you wish to have. Please visit the NYS DMV website https://dmv.ny.gov/which-id-right-me for specific instructions and information about requirements. All applicants must have the MV44 form completed prior to your appointment, please bring this with you along with the applicable documentation.

IMPORTANT - IF YOU TAKE YOUR PERMIT TEST ONLINE, YOU HAVE TO UPLOAD YOUR DOCUMENTS PRIOR TO TAKING THE TEST. YOU STILL HAVE TO COME TO A DMV OFFICE TO FINALIZE YOUR PERMIT AND SHOW ALL OF YOUR DOCUMENTS!!! You do NOT retake the test!!
