This is an approval form for those who have previous work done (micro shading or microblading) from another artist and would like for me to consider them for my cover up/correction service.

-Answer all required questions
-Submit all required photos
-Await response on approval or denial

*If your brows are approved, you'd then be given directions on how to schedule your ombre brow correction session with me.
