We are so happy you have decided to schedule a nutrition visit with us! Just a reminder, we will ONLY be meeting via Telehealth for ALL your visits. Most insurance companies are allowing/covering all telehealth visits. Be sure to ask them about coverage when you verify your benefits.
If you are a NEW client using INSURANCE, please select the option for a INITIAL Visit: Insurance Pay.
If you are a NEW client paying out of pocket/cash pay, please select the option for INITIAL Visit: Cash Pay.
Please Note: All Cash Pay Visits (Initial and Follow-up) will require a 50% deposit upon scheduling to secure your appointment time.
If you have been seen by one of our dietitians in the past 6 months please select the option for a FOLLOW-UP Visit.
