This is the initial intake for all chronic pain patients to this practice. If you have never been to Arizona Pain & Injury or Injury Medical Group then this is the appointment you need. Payment is due at the time of scheduling. At your request we will provide you with what you need to attempt to be reimbursed by your insurance company.

Online encounter to assess response to prescribed pain medication or other controlled substance. There is no guarantee that medication will be renewed. Must be an established patient to use this service. Answer questions online to submit request. Request is processed within 72 hours. For same day processing you may select the expedite add on for an additional fee.

In office, follow up appointment for patients who are established at Arizona Pain & Injury or Injury Medical Group only. If you are transferring from another practice you are a new patient. At this time insurance is not an option for payment. Payment is due at time of scheduling.

This is a follow up encounter that may be done via remote interactive video or by telephone or other electronic means. This is a medical encounter appropriate for medication refills, treatment planning, referrals management, and assessment of response to current treatment. Not recommended for new complaints.

Routine follow up appointment with provider for established injuries from auto accident, slip and fall and other injuries that are not work related. This is a remote visit by interactive video or telephone

This is for established work related injuries in person. Arizona requires monthly follow up with all treating providers. This satisfies that requirement. Do not recommend for routine items like imaging review or medication refill.

Routine follow up and continued treatment reporting for injured workers. Not recommended if complications or treatment failure. Good for medication refills, imaging review, referrals management, other routine needs. Visit is by remote interactive video or telephone
