We understand that everyone's time is valuable and things come up, but each time a client misses an appointment another client is prevented from receiving care.
CANCELLATION POLICY AGREEMENT
I understand when cancelling or rescheduling an appointment, I am required to do so no less than 24 hours prior to the scheduled time of my appointment. In failing to do so, I acknowledge it will incur in one of the following charges:
If cancellation/rescheduling occurs between 24 - 12 hours prior to appointment, I will be charged a 50% of the service fee + a processing fee, if the cancellation is made within 12 hours of the appointment, the fee will be 100% of the service fee + processing fee. This 100% service fee + processing fee will also apply to anyone failing to show up to their appointment.
HEALTH INSURANCE: If you have acupuncture benefits with your Health Insurance, you will be responsible for any of the above fees. Insurance plans DO NOT cover Cancellations/No show fees.
CONTINUATION WITH BOOKING AN APPOINTMENT, YOU ARE AGREEING TO THE ABOVE CANCELLATION AGREEMENT.
