A parent or guardian must be present at the vaccine appointment if the child is under the age of 16.
ITEMS TO BRING: Government Issued ID, Insurance Card & COVID Vaccination Record Card if this is not your first dose. (4th shot only for people that received the additional 3rd dose for they are moderately/highly immunocompromised and are eligible for a 4th dose at this time.)
