COVID-19 VACCINE REGISTRATION
Ottawa County Department of Public Health Clinic: April 27th, 2021 (Pfizer)
2nd dose appointments will be on May 25, 2021
If you can not make both dates, do not make an appointment.
16 & 17 youth may only receive Pfizer. Parental consent is needed and date of birth proof is required (driver’s license, state ID or birth certificate). If an appointment is scheduled for someone younger than 16 years, the appointment will be canceled.
Holland Hospital - Urgent Care Clinic: April 26,
2021 thru April 30, 2021 (Moderna)
2nd dose appointments will be May 24, 2021 thru May 28, 2021
You will be expected to return on the same day of
the week as your first dose.
If you can not make both dates, do not make an
appointment.
Please note that your may receive a reminder text or email that states: State of Michigan COVID-19:2nd dose COVID-19 is due on a specific date. Please disregard and come at your scheduled appointment time.
REGISTRATION CONDITIONS:
- Ottawa County is currently administering first and second vaccine doses to all eligible adults 18 and older and youth 16 and 17 with Pfizer only.
- An appointment is required and is non-transferrable. No walk-in appointments are allowed.
- Do not schedule an appointment if you:
- Received any vaccine in the past 14 days,
- Are in isolation or quarantine, or
- Have received monoclonal antibody therapy for COVID-19 in the prior 90 days.
- Consult your healthcare provider prior to registering if you are pregnant or lactating.
- All 2nd dose appointments (Pfizer & Moderna) will be scheduled between 28 & 31 days after the 1st dose. This timeframe falls within the CDC guidance.
REGISTRATION INSTRUCTIONS:
- Select date and time below for your appointment. If no times appear, no appointments are currently available. You will be notified as more appointments become available.
- Complete registration form.
- Use your legal name (as written on a driver's license, state ID, birth certificate, visa, passport, etc.).
- For appointment confirmation and reminders please provide an email address.
- If you do not have an email:
- Type "none" below,
- Note the location of your appointment on the confirmation below, and
- Note the appointment details below (you will not receive additional information prior to your appointment).
- To receive a text message reminder, please provide a cell phone number.
APPOINTMENT DETAILS:
- The location of your appointment will be confirmed below upon completing registration.
- If an email was provided, a confirmation will be emailed along with these instructions.
- When you arrive, wait in your vehicle until your scheduled appointment time.
- Please do not arrive early. If you have an appointment, you will receive vaccine.
- Appointment will take about 30 minutes.
- What to bring to your appointment:
- Mask (required to be worn to enter building)
- Photo ID (required)
- Cell phone (if you have one)
- Reading glasses (if needed)
- Wear clothing that allows vaccine administration in your upper arm
- Vaccination card (if this appointment is for your second vaccination dose)

All appointments are based on the availability of vaccine supply from the State of Michigan. You may be notified to reschedule your appointment if vaccine supplies are not available.
