After booking your desired treatment, click the 'Book Another Appointment' button at the bottom of the treatment confirmation screen, then select the First Session Intake Form and book it like an appointment. Select a time about 5 minutes right before or immediately after your booked treatment. If there is no available space to do this, just book it as close as possible. After completing the form don't forget to click 'Schedule Appointment' to submit
