SCHEDULING YOUR APPOINTMENT
- Do NOT schedule an appointment until you have all of your tax documents (SEE What to Bring BELOW) otherwise your appointment may be rescheduled
- If filing jointly, taxpayer and spouse must attend appointment together
- If arriving more than four (4) minutes late, appointments may need to be rescheduled
- Schedule only ONE appointment per taxpayer. Later, if you need to reschedule, please cancel your original appointment

A. Download, complete and save (virtual) or print (drop-off) the following forms (click blue links) and bring ALL completed forms with you to your scheduled appointment:
- Intake Form 13614-C
- Consent Form 14446 (choose your tax appointment site only)
B. Virtual VIP via Zoom ONLY
C. Drop-off Sites ONLY
- For the health and safety of the community, all clients will be required to answer a COVID-19 screening questionnaire and must comply with COVID-19 safety protocols (wear a mask, watch your distance, and wash/sanitize your hands).
- Only taxpayer(s) being serviced will be permitted to access the site.
- Follow the on-site instructions and signage regarding site access.
- NO children allowed at appointments.
- Bring a copy of the Hold Harmless Waiver
- Hard copies of ALL of the above forms are available curbside at some public library locations. If you have questions about how their curbside service works, please contact the local library:

Due to the uncertainty around the COVID-19 global pandemic, operations of the sites are subject to change without notice.
D. What to Bring: ALL of your tax, ID and SS documents including:
- Valid government issued photo ID for taxpayer and spouse (if married filing jointly)
- Social Security cards for ALL individuals to be listed on your tax return
- W-2s and 1099s from ALL your employers (no pay stubs accepted)
- List and/or documentation of other income/expenses, including mortgage interest and property taxes paid during the year
- For direct deposit to an existing account, a blank check or account ID card with the account number and routing number information
- 1095 A/B/C for Proof of Health Insurance Coverage
- 1098 for student loan, mortgage interest information
- 1099s for interest received, dividends received, unemployment compensation, pension and/or other type of income
- Copy of prior year tax return (may be required for new credit)
