INSTRUCTIONS FOR VACCINATION APPOINTMENTS ONLY:- You must be 18 years of age or older to receive the Moderna vaccine.
- You must be free of symptoms (fever, cough, sore throat) and not be in isolation or quarantine to receive a COVID-19 vaccine.
- Remember to wear short sleeves or clothing with easy access to the upper arm.
- Please wear a mask.
- Please leave pets at home.
Your second dose appointment for the vaccine will be scheduled for you by a staff member. You do not need to schedule your second dose appointment at this time. You will be notified via email or phone regarding the details of your second dose appointment after you receive your first dose.
