Welcome to our online COVID-19 Vaccination Clinic scheduling. You must be 65 years or older for this clinic.

• This is a 65+ Clinic, you will need to have proof of your date of birth. (drivers license or other form)
• Do not come early; space is limited. Do not bring anyone inside with you unless you need assistance.
• Wear a mask and keep 6 feet apart from others.
• You will be observed for 15 minutes after your vaccination so plan on your appointment lasting 25-30 minutes total.
• Please cancel your appointment if you are sick or unable to attend.
• Please do not schedule an appointment if you have had another vaccine within the last 14 days.
• If you had COVID-19 illness AND received passive antibody therapy (plasma), within the last 90 days, do not schedule an appointment, vaccination should be deferred 90 days from treatment.
