COVID – 19 Vaccine Clinics
When scheduling your COVID-19 vaccine appointment, please be aware of the location of your appointment.
- If it appears that a clinic does not have any dates to select, that means the clinic has no available appointments and is full. Clinic availability may change due to appointment cancellations.
- Covid vaccine clinics will either be held at the Onawa Community Center or the Mapleton Community Center. Please be aware of your appointment location.
- Bring your state-issued ID card and Insurance card if available.
- The shot will be given in your upper arm/shoulder area. Please wear short sleeves.
- Only schedule yourself for one appointment, and only one person/one name per slot. If you are receiving a two-dose series, you will be scheduled for your second dose when you receive your first dose.
- You will be provided a vaccine card when you receive your first dose. Please keep and bring this card back with you for your second dose.
- If you have any concerns about health conditions and if you should receive the vaccine, please discuss this with your health care provider before scheduling your appointment.
