The COVID-19 Vaccine is available to people 16 years and older in WA.
Our Duvall location has the Moderna products which are approved for 18 years and older.
First Bivalent boosters available to patients 18 years and older.
Additional Bivalent boosters are available to patients 65 and older, or those who are immunocompromised.

Once you schedule an appointment, an email with instructions and a link to complete the online vaccine consent form prior to your appointment will be sent. Please fill it out as soon as you get it, so we can be prepared to vaccinate you when you arrive.
If you cannot access the form, please show up to your appointment 10 minutes early to complete one. Be sure to bring your medical and prescription insurance cards with you. If you are covered by Medicare, please bring your red, white and blue Medicare card with you.
