Your fingerprints will be rolled onto a fingerprint card that you take with you at the end of your appointment. Customers may bring a fingerprint card that has been provided to them, or we can provide Form FD-258 (with the Privacy Act Statement). Form FD-258 is accepted by most states in the U.S. (if you are an out-of-state applicant), the FBI & other federal agencies, or a for foreign passport application.
More Than 5 Paper Fingerprint Cards Needed?
If you need to complete more than 5 paper fingerprint cards, before scheduling an appointment, please send us a message in the Contact Us section on the homepage. Appointments with a large quantity of fingerprint cards may need to be scheduled outside of our normal business hours so that other customer's appointments are not displaced.
What ID's are acceptable?
Non-expired, government issued photo IDs from ANY state or country; e.g. driver's license, passport, or passport card. For customers under 18 years old a photo school ID is acceptable.
Payment
Payment is due by credit or debit card at the time of the appointment. No cash or check accepted.
