BY BOOKING: I, the client, am declaring that I understand there are some risks associated with having artificial eyelash extensions applied to and/or removed from my natural lashes, and that not withstanding the utmost of care in the application or removal of these products, there are still risks associated which include, without limitation, eye irritation, eye redness, discomfort. irritation of skin (for waxing procedures), and in very rare cases, blindness when improperly handled. As part of this procedure, I understand that a certain amount of eyelash adhesive material will be used to attach the artificial lashes to my natural lashes. Despite professional applying or removing material correctly, some material may become dislodged during or after the procedure, which may cause redness or require further after care instructions, at my own expense, to prevent eye damage. I also understand there are multiple techniques for applying eyelash extensions and I will not attribute any liability to the eyelash extension professional as a result of my eyelash procedure and also for any waxing purposes. Lastly, I agree to defend, indemnify, and hold harmless the eyelash extension professional from any and all claims, actions, expenses, damages, and liabilities, including attorney fees which might be asserted against said professional as a result of any procedure listed below.
