Membership Terms and Agreement

By enrolling in the Transformed Membership, you agree to the following terms and conditions:

1. Membership Commitment: This subscription requires a 2-month commitment. Early cancellation is not permitted unless under exceptional circumstances, which must be discussed and approved in writing.

2. Payment Terms: Membership fees will be charged monthly, with payment due on the same date each month. Payments are non-refundable and services must be used within the billing cycle. Unused services do no roll over.

3. Membership Freeze Policy: Members may freeze their subscription for up to 2 months per 12 month period without penalty. Requests to freeze must be made in writing at least 7 days prior to the next billing cycle. During the freeze period, no membership fees will be charged, and services cannot be used.

4. Scheduling: Appointments must be scheduled in advance and are subject to availability. Clients are encouraged to schedule sessions regularly to maximize the benefits of the subscription.

5. Cancellation Policy: Appointments canceled or rescheduled with less than 24 hour notice will be forfeited and cannot be rescheduled. Emergency cancellations will be reviewed on a case-by-case basis.

6. Service Usage: The membership includes Yates Aesthetics Signature Services: Full Body Lymphatic Drainage, Fat and Cellulite Reduction.
Services cannot be transferred, shared, or substituted for other offerings.

7. Health and Safety: Clients must disclose any health conditions or recent illnesses prior to services. If a client is feeling unwell, they must notify the service provider and reschedule their appointment to protect the health of both parties.

8. Termination: Membership automatically renews after the 3-month period unless canceled in writing prior to the renewal date. The provider reserves the right to terminate the membership if policies are not adhered to.

9. Liability: The provider is not responsible for any adverse reactions or outcomes if clients fail to disclose medical conditions, allergies, or other health concerns.

By signing below, I acknowledge that I have read, understand, and agree to the terms of the Transformed Membership.
