You may schedule your Covid Vaccine appointment via this website for Pfizer ages 5-11 years and 12+ years, and Moderna all ages.

If you want to schedule your child's 6 month - 4 year Covid vaccine, please call our office to schedule as supplies are limited for this age.

When registering, please use the name of the person receiving the vaccine. If multiple family members (including parents!) are receiving vaccines, please schedule an appointment for each person. If you or your child requires multiple vaccines, such as Flu + Covid, please schedule an appointment for each vaccine.

Multiple appointments made under 1 name will be cancelled and asked to schedule properly.

We maintain the right to deny vaccines for unscheduled persons.
