I understand the risks of my intended service.

I understand that after the procedure I may experience eye irritation, itching discomfort, and in very rare cases an eye infection may occur.

I understand that some extensions may fall off. I have 48-72hrs to contact the technician to have them fixed or replaced without a fee.

I understand that this is a NON-REFUNDABLE LASH SUBSCRIPTION. If I wish to cancel the subscription I must contact my technician.

I understand that I will be charged the full amount of my chosen subscription every month unless I cancel it.

I understand that a 10 min grace period is given for every appointment anything later will result in a $15 late fee or cancellation. This means on the lash bed before the grace period is over

I understand if I no call no show your subscription will automatically be canceled and you will blocked and banned from all bookings

I understand that if I fail to follow all Policies or procedures, Kayla with Monè Artistry has the right to refuse any service.
