PLEASE READ! Please note that this form is for Pediatrics East patients ONLY. Please submit The Lactation Network (TLN) or Wildflower Health form BEFORE scheduling an appointment below with Brandi as you will need to know if your insurance works with TLN or Wildflower Health or not in order to choose your appointment type. Many families are covered 100% for their lactation visit with no out of pocket cost.

If you do not have health insurance, please proceed with scheduling below. If your insurance declines to work with TLN or Wildflower Health, then a superbill can be provided to you to request insurance reimbursement.

If you chose a home visit on your TLN or Wildflower Health form, please email support@midsouthlactationconsultants.com to schedule. Include your name and specify that you are a Pediatrics East patient.

Please note that this form is for Pediatrics East patients only who wish to schedule a lactation consult with Brandi Jordan at Pediatrics East. Brandi is currently at the Bartlett Pediatrics East office on Tuesdays, the Germantown Pediatrics East office on Wednesdays, and the Arlington Pediatrics East office on Thursdays. If you wish to schedule with Cindy on Fridays at the Collierville office, please call 901-757-3535.
