I agree to have eyelash extensions applied to my natural lashes and/or removed, and retouched.By booking an appointment, I consent to the placement and/or removal of the eyelash extensions by the eyelash extension technician at DeeLash Pad LLC.
I understand that in rare occasions there are risks associated with having artificial eyelash extensions applied or removed from my natural lashes. I further understand that in rare cases as part of the procedure eye irritation and discomfort could occur. I agree that if i experience any of these conditions with my lashes I will contact the certified eyelash extension professional that performed this procedure and it may be beneficial to have them removed.
• There is a $20 deposit required to book that will go towards your appointment and a $3 convenience fee. Deposits are non-refundable and not transferable .
• There is a $30 surcharge for all squeeze in and after hour appointments . *Sundays can be accommodated for this fee, please txt 773-424-2426. This fee applies to all appointments not booked through the booking site .
