Please answer a few questions to help us get started with scheduling your appointment at SAMP Labs.
Scheduling via Phone or Other Types of Testing
At SAMP Labs we offer many types of testing that require additional information, like blood testing, if you are looking to schedule an appointment for other types of testing, please contact us at testing@samphealth.com or you can call us at (440) 297-4100.
Our Team is available Monday through Friday, 9:00 a.m. to 5 p.m. (Eastern Time).
What to Bring:
1. Valid Driver’s License
2. Proof of Valid Medical/Health Insurance for current calendar year
3. If Self-Pay - Please bring Cash and/or Credit/Debit Card - No Checks
When you arrive:
For RT-PCR Covid Appointments:
1. Park in the west parking lot
2. Stay in the car, Call
3. Call the Lab at (440) 297-4100, to speak to a Team Member
4. If your time has passed, please call the lab (440) 297-4100
For All Other Appointments:
1. Enter Lab, Suite 101 to Check-in
2. Please arrive 10 minutes before your appointment
3. If you are late, we will prioritize customers who are already in the laboratory first.
