An overall available schedule is given, however, that may not work with their Life in that moment. The Recording Secretary may request more information to present Before the Call. See below for items that will be requested and how to send. The Recording Secretaries are living men and women and Life can change quickly.

Witnessing/Recording Calls are charged once the appointment has been scheduled.

  • For Witnessing/Recording Appointments, email the following items to records@TheIndianaAssembly.org to receive approval for this appointment:
    • Scanned PDF copy of valid photo ID/DL with current Indiana Home address - if address is not current, still provide the Valid Photo ID/DL and recent piece of mail with the same Name that does have the current Home address.
      • Note: To be recorded by an Indiana Assembly Recording Secretary, it is required that you are currently Living on Indiana Soil-County. If you live on Any of the other 49 States, you are to present to that State's Assembly. US Territories and POSSESSIONS are Not one of the States, nor can you 'Adopt' a State in this case. You will have to go to the US Embassy and declare your Status that you are Not a US Citizen/citizen of the US.
    • Scanned PDF copy of Birth Certificate, and any supporting documents - short form acceptable, does not need authenticated, unless you plan to travel outside of America.
  • If your 1, or both, of your witnesses are present for the Recording Secretary to Notarize their testimony, witnesses will need to present a valid/current photo ID/DL with their current Home address, can present this into the camera on the call. If ID/DL does not have the current address, a current piece of mailing received with the 'Name' on the ID/DL.
    • And are Required to meet the following conditions per Article 1598 Minnesota Rule:
      • 2/Two Living Men, and/or Women, preferably over the lawful age of 21, AND
      • Have Known you for a minimum of 7 years

The above items are requirements to verify who you are and where you call Home today. Call will not be Approved without providing the above verification. Notarized Witness Testimonies do Not need to be presented before the call, nor are your witnesses required to be on the call. The Witness Testimonies are prepared for either a STATE OF INDIANA Public Notary, or an State Assembly Recording Secretary. The witnesses with sign as they normally do, include type of relationship and length of time known-ex. partner 12 years, or natural mother, or step father for 24 years. Public Notary does NOT need to add a second 'Notary Page,' and does NOT need to alter the Notary block.
