If you live outside this service area, we may be able to see you for an additional charge. In this instance, please book a WAITING LIST appointment.

What is a convenience fee?
It is a concierge fee charged to have the doctor travel to your home. The visit itself, plus any medications, testing, etc, will be filed to your insurance.

Fill out the intake form to have your name placed on our waiting list. You'll be asked to give a time frame that you'd be available. If a spot opens up, you'll receive an email confirming your appointment time. If you haven't received an appointment within 2 hours of placing your name on the list, it's unlikely we will find an opening today but we will still keep you on the list, just in case! If you're adding your name to the waiting list in the middle of the night, we'll check the list in the morning!
