SERVICES

We provide eye examinations for glasses and contact lens prescriptions for ages 4 and older. Children should be familiar with letters and shapes for best results.

Our office is handicap accessible. We also screen and provide treatments for most eye issues including, but limited to eye infections, dry eyes, teary eyes, itchy eyes, glaucoma, cataracts, diabetes, and LASIK referrals.

OFFICE SPACE

Our office is a very small space. We are handicap accessible, but larger families and parties may need to come in two at a time. Minors or patients who require assistance can bring 1 guest with them. All other guests are welcome to wait nearby. We appreciate your understanding and apologize for any inconvenience.

After you have chosen your appointment time, please consider filling out your paperwork at home. Even if all your information is the same as previous visits, please re-enter this information to confirm, especially changes to your health history.

Vision Insurance

We are accepting some Medical Insurances at this time, see below.

STEPS TO CONFIRM YOUR ELIGIBILITY

1. After choosing your appointment time, fill out and submit all appointment info sections

2. Call, Email, or Speak With Staff about your copay and eligibility status before the appointment.

3. Confirm with staff if you have a separate copay for Contact Lenses Exam and Fitting

Don't forget to schedule your family members, even if they do not need glasses for driving or had LASIK or PRK corrective eye surgery. Your vision insurance can help pay for Sunglasses, as long as you have a prescription.


Vision Plans We Accept

Our office is independent of Costco warehouse, so we accept some but not all of the same insurances listed on Costco's website.

We are NOT providers for Blue View Vision, Eyemed Vision Insurance, Davis Vision Insurance, Superior Vision

Please call us to verify that we are in-network providers for your Vision Insurance. We are in-network providers for:

AETNA with Vision Service Plan (VSP)

Some AETNA Medical Insurances (Call to Confirm)

We are In-network for most, but not all VSP PLANS, A small percentage of VSP plans are considered "OUT OF NETWORK" at retail opticals, such as COSTCO and independent optometrists adjacent to COSTCO. Please provide your information and we can confirm your elligibility and copay.



Vision Insurance versus Medical Insurance

Vision insurance covers routine eye exams for glasses and sometimes contact lens fittings. Medical Insurances, when accepted, does not cover eye exams for glasses and contacts

Your medical plan may have a different name than your supplemental Vision Plan. Please check your card, ask your HR rep, or call your medical provider to determine which vision plan you have. Please call us before your appointment to confirm insurance eligibility, copay amount, and benefits.

If you would like to use vision insurance for your visit, please provide us with all the required information on our online in-take form, so that we can authorize your benefits prior to your appointment. We will not be able to apply insurance benefits if some of the information is not provided IN ADVANCE.

Currently, the only medicare insurance we are in-network with is AETNA. If you have a PPO insurance plan, we can provide a receipt with all the medical codes you will need to self-submit an insurance claim. If you have the ability to file a claim to be reimbursed by your medical insurance or your employer, we can provide a itemized receipt with medical codes. Unfortunately, Medicare does not allow self submissions for reimbursement. Please, call us at (703) 365-0500 if you have any questions.
