What to bring to your appointment:
- Every person (including children) applying for a passport must attend.
- All parents/guardians of minors (under 16 years of age) should attend. A Form DS-3053 Statement of Consent may be used if a parent/guardian cannot attend. This form must be notarized and accompanied by a photocopy of the front and back of the absentee parent/guardian's ID.
- Bring a completed DS-11 passport application form. Forms may be printed from travel.state.gov (copies are available at the Library). If handwriting the form, use black ink. Do not sign the form until you meet with us.
- Bring original proof of U.S. Citizenship and a photocopy (certified U.S. birth certificate, certificate of naturalization, certificate of citizenship or a previously issued U.S. passport). Your original proof of U.S. citizenship will be returned to you in a separate mailing.
- Bring proof of identity and a photocopy of the front and back (driver’s license, passport, state ID card).
- Bring one recent 2" x 2" color photo. Please click here for photo requirements. The Library provides photo services for an additional $14 fee.
- Bring at least 2 checks or money orders. One will be made out to "The Smithtown Library" for execution fees and photo fees as needed. The other(s) will be made out to the “U.S. Department of State” and attached to each application. Click here to calculate fees and view the current fee schedule.
Note:
- The Smithtown Library is authorized to accept passport applications on behalf of the Department of State-Bureau of Consular Affairs, Office of Passport Services. All passport decisions will be made by the Bureau of Consular Affairs.
- This service is only available at our Nesconset Building.
- Save time by bringing photocopies of IDs and proof of citizenship for each applicant.
