No Show Policy

• In order to keep my rates low, I have a no show policy of a nonrefundable charge of $50.00. I do not make any exceptions.
• Cancellations are fully refunded when made more than 24 hours in advance. All appointments canceled within 24 hours of the appointment time (for any reason) are considered no-shows. Need to cancel? Do so through the Acuity Scheduling program under “Book Now”.

Paperwork Needs to be Signed Before Treatment Starts

• This paperwork (one page) will be emailed to you and needs to be signed before treatment starts. It can also be filled out during the appointment if the appointment is in office.

I do not conduct therapy over text message or email.

An HIPAA BAA agreement is in place with Acuity Scheduling and this is HIPAA compliant. You have a right to request this agreement before entering PHI into Acuity Scheduling and use of Acuity Scheduling is not required for services. Feel free to call/text me (520 239 6623) if you prefer not to provide PHI to Acuity Scheduling.
